Day 1–2: Tbilisi old town and modern city
Arrive in Tbilisi and spend the first afternoon walking the old town: Meidan Square, the sulfur bath district (Abanotubani), and the cable car up to Narikala Fortress for the sunset. Eat dinner at a wine bar on Erekle II Street.
On day two, see the Holy Trinity Cathedral, walk Rustaveli Avenue end to end, and visit the National Museum for the gold collection. Finish with a banquet at a traditional supra restaurant like Shavi Lomi or Barbarestan.
Day 3: Day trip to Kazbegi and Gergeti Trinity Church
Hire a driver or take a marshrutka 3 hours north along the Georgian Military Highway. Stop at Ananuri Fortress, the Russia–Georgia Friendship Monument, and the Jvari Pass. In Stepantsminda, take a 4×4 or hike 1.5 hours up to Gergeti Trinity Church for the postcard view of Mount Kazbek. Return to Tbilisi by evening, or stay overnight if you want to hike the next morning.
Day 4: Mtskheta and the road east
In the morning, drive 30 minutes north to Mtskheta — the ancient capital and a UNESCO site. Visit Svetitskhoveli Cathedral and the hilltop Jvari Monastery. After lunch, head east to Sighnaghi, the walled "town of love" in Kakheti, and check into a guesthouse with views over the Alazani Valley.
Day 5: Kakheti wine country
Spend the day visiting Bodbe Monastery, the cellars of Telavi and family wineries around Kvareli. Try Saperavi, Rkatsiteli and traditional qvevri wines. Pheasant's Tears in Sighnaghi or Schuchmann Wines outside Telavi are reliable choices. Dinner: a Kakhetian supra at your guesthouse.
Day 6: Back to Tbilisi, modern art and rooftops
Drive back to Tbilisi in the morning (2 hours). Spend the afternoon in the Vake and Vera neighbourhoods — coffee, vintage shops, and the MoMA Tbilisi. Climb up to Mtatsminda Park by funicular for sunset over the city. Dinner at one of the natural-wine bars in Old Town.
Day 7: Departure (or extension to Batumi)
Use the last morning for souvenirs at the Dezerter Bazaar or a coffee on Fabrika rooftop before heading to the airport.
Extension: with 2 more days, take the morning train to Batumi (5 h) for the Black Sea coast and Adjarian food; with 3 more, fly or train to Mestia for hiking in Svaneti.
